One possibility if:

1. You live within a few miles of the phone exchange.
2. You want moderate speed internet. Meaning 10Mbit or so. Enough for HD television.
3. You want a real landline with all features. Meaning:
a. Three way calling
b. caller ID
c. unlimited US calls
d. 8 hours per month of calls to certain countries (Canada, Australia, UK, Japan..)
e. A fixed IP address.
f. Reception of faxes without a machine. (They convert to PDF and email them)
g. The usual website with tools, etc.

About $56 per month flat, including taxes and lease of a wireless router.

NOT including TV. But if it fits try Sonic Telecom

Only in California though...
