The facts the OPRRMS has stated appear to be completely factual as it was also related to me. The purpose of the investigation is as Sam stated. Robert the timing issue is being analyzed as we speak.
Since I don't have a conflict with any of these client agencies at this time, as a few gadlys have asserted, I don't have any professional obligation to mute my opinion. My gut feeling is beginning to go with the unfortunate preponderance of evidence, and the other possibilities for non human failure cause are getting thin. I don't give much credence to the rest issue, that's not that grueling of a weekday daytime schedule for an experienced hoghead.
The ML engineer blew through an absolute red, and a mainline switch lined against him, after passing flashing and solid yellow approaching block signals, while running a passenger train, and didn't take any actions that would indicate he even noticed? Barring something dramatic in the engineering analysis, pathology, or a finding that the reported distracting activities did not fit the time frame, this increasingly apparent explanation, may also, unfortunately, well be the most plausible.
