In my opinion, I think that what would have prevented Chatsworth outright was having another person in the cab, ie conductor, fireman, ect. two sets of eyes are always better than one, and if there had been another person in the cab they could have told the texting engineer to put the damn phone away and pay attention. The second person would have also have said something before the engineer ran a red and forced open a switch set against him. Is a second person perfict? Of course not, there have been MANY cases where both people in the cab feel asleep, but I still say that having two people in the cab is safer than one.
