Gauge is a relevant issue as SMART-NCRA-NWP is of course not 5'6'. Geary is one of the ways for rail to access the GG Bridge and since the current version of the Central Subway looks to have some serious built-in operating deficiencies perhaps a Northbay connection to a Geary corridor subway would be the best way to go.
Outer Geary will be a challenge to lay out for surface rail. It is a traffic and pedestrian nightmare. As grotesque and visually clumsy as it may sound pedestrian overpasses to a center island might be the safest way to proceed.
Altho the truly high passenger count on Geary cries out for rail, I still think conversion straightawy to trolley coach makes a lot of sense. The deadhead savings should be substantial with the yard right on the line. You would probably have to expand Presidio bus barn somehow.
Of course, in a perfect world a reborn B with curb lane rails and 3-section articulateds all on the surface could make a lot of sense but Geary and O'Farrell downtown are wacko. Double-parked trucks, clueless space cadet pedestrians darting out from nowhere, yada, yada.
