In Europe, in most cases, someone high up enough in the
govenment makes the decision to build a high-speed line
-- and it gets built. European countries have plenty
of environmental regulations and concerns, but these do
get taken care of once the go-ahead decision has been
made.
One thing that has helped is that most of Europe's rail
networks are electrified, making it easy for the high-speed
equipment to operate into existing stations or to detour
when necessary.
I once interviewed an official at Siemens who had come to
the U.S. after many years in Europe. He said he was
amzaed that it took "forever" to get anything rail-related
built in the U.S.
