"Tyrrell said she did not know why a series of safety measures and controls along the way, including communication with dispatchers, failed.
She said the engineer, whom she did not identify, was a subcontractor with Veolia Transportation and a former Amtrak employee. Authorities later confirmed that he was killed in the crash.
At a news conference about two hours after Tyrrell's remarks, National Transportation Safety Board officials cautioned that the cause of the collision remained under investigation.
"We don't know why the accident happened," said Kitty Higgins, an NTSB board member. "Our process is to look at everything before we rule anything out."
Fourteen of the NTSB's investigators working the crash scene had recovered data and video recorders from both trains, she said. Those devices should allow investigators to determine information about the trains' speed and brakes, the mechanical signals and when the last time the horn was blown.
In addition, investigators had interviewed the surviving crew members of both trains. Higgins said four of the five crew members between the Metrolink and Union Pacific trains had survived and had been interviewed by investigators."
