Was he alone in the cab? The companies and the unions have negociated the second set of eyes away,and imagine,only on passenger trains. ALL freight trains in North America have two people in the cab.
Was a cell phone or other wireless device in use. Alot of incidents in the last 10 years have happened due to the Engineer using a personal or company wireless device. The last two Metra accidents in Chicago where fatalities were involved were due to the Engineer loss of focus onhis duties because of a cell phone.
Was he,and I'm calling his gender because of knowledge,in complete good physical and mental shape. Companies no longer subject their employees to the annual physical that used to be the rule. I believe medical costs and right to privacy issues come to play. There was a time you couldn't get a railroad position such as brakeman or fireman without being in perfect physical condition and have 20/20 eye sight wiyhout glasses. Was he suicidel?
Was he on any controlled substances? Not only Alcohol or drugs but possibly some presdribed substance for other problems.
A lot of varibles can come into play when a terrible incident takes place. Usually there will be at least ten things that lead up to the incident.
