Well, they can't be everywhere, but it seems to me that there had to have been some kind of warning signs that the Chatsworth engineer was completely neglecting his duties. This can't have started that same day. (I assume, by the way, that if the regular conductor on that train was not following rule 1.4 to report rule violations -- that the engineer was not calling signals -- he's no longer working for Metrolink's contractor. At least, I hope not.)
By the same token, it's hard for me to imagine that two people who not only saw the signal indications but must have been -- or certainly should have been -- visually aware of the BNSF train just got that way all of a sudden on a clear, sunny day. Somebody should have seen some kind of warning sign. Or does everyone cover for everyone else?
