Regarding the Rialto Metrolink collision:

The conductor had been observed on 32 separate days between November 2008 and November 2009. There were 123 specific observations recorded. Eight noncompliance events were noted: . . . his name tag on his uniform was missing once.

The fact that NTSB investigators would even include this tidbit of information in an ACCIDENT REPORT speaks volumes of where we're at in the railroad industry today. In the case of taxpayer funded-commuter operations, we have a bunch of clueless politicians micro-managing and trying to "play with trains." A conductor without a name tag is apparently a really big deal, so much so that data must be collected and tabulated on such an event.

Simply put, they (Metrolink managerial personnel and NTSB investigators) look foolish accumulating such data and figuring it is necessary to include it in an accident report.
