#1 is a no brainer that the crossing needed to be flagged if the signals were not working properly.
#2 would be a rules violation at the rr I have volunteered at as well. Rule books there require "stop and drop" meaining if you are going to back through a crossing not protected by gates and or lights you must stop and drop off a flagman before backing through the crossing. A flagman could arrive at the crossing by road and already be in place eliminating a stop, but regardless of how it is accomplished a flagman is required. I don't know GCOR rules so no idea if this is a pretty universal requirement? Also the locomotive whistle would be blown for the crossing even in a backing move until the crossing is fully occupied just like a forward move. The peanut horn on a monkey tail would only be good for warning pedestrians in my view.
