In the dark, if you don't really know where you're going or are otherwise confused, it's quite possible to end up on the tracks. Especially where they are very close to an actual cross street.
There's a poorly lighted light rail station entrance I've used often that crosses an industrial spur. On a foggy dark morning it's quite easy to miss your lineup with the driveway and end up stuck on the tracks. Almost did it once; luckily, no traffic that early so the odd correction down the road shoulder didn't bother anybody.
Mistakes can and will be made. I won't say it's not the guy's fault (as his mother did to the reporters) - if you're driving and you make a mistake causing a problem, sorry, it *IS* your fault. You were supposed to know better. But us chimps ain't perfect. Hullo Apple/Google (given the mess Microsoft made with Ford's Sync setup I don't think I want them doing anything more important with my car)?
