These were very common into the early 70s - an integral part of the hospitals where I worked in the 1960s. I could pick up the phone and ask the operator for a line to the geographical area I wanted to call and get it. Being in Norwalk, California and being able to just ask for an Orange County telephone line and then directly dial the call - toll free, yet - was quite something.
