In the glory days of the PRR, they used " Smash Boards " at VERY important signals that had been run through in the past. If the signal was red, an oversized tennis racket " thingie " dropped down over the tracks. A excellent picture of one appears on Pg. 61 of Robert Yanosey's " Penn Central" Power book.
But all these signal devices are only a patch; the engineer still has to PAY ATTENTION to them. This is why I believe some sort of PTC / ATS system, that operates INDEPENDENTLY of the engineer, will soon be mandated for high density trackage with passenger trains.
