Looking at it initially, I thought it was a Kodachrome 200 slide. Thankfully that film didn't come on the scene until about 1986 and it only lasted about 10 years. It was very grainy compared to KR64.
With a difficult image like this, I suggest you try Vuescan's excellent support for RAW format scans. Scan it as a RAW file then you can try different post-processing settings to get the best results without having to scan it over and over. I don't use sharpening at all in Vuescan and instead use Smart Sharpen in PS. I use Noise Ninja as well and it usually works very well for me with difficult scans.
Give RAW a try. I don't know what kind of scanner you are using Vuescan to drive but try keeping it under 4,000 ppi as well.
