I've never made a claim that a second person in the cab would guarantee safe operations 100% of the time. What I
am saying is that a second set of eyes and ears offers more than a single set of eyes and ears. Even the NTSB theorized that a second person in the cab probably would have told Sanchez to knock off the texting.
As to the Rialto incident, I don't know why the second person didn't speak up and say anything. I still wonder whether a second person
really was on the engine. It could have been "damage control" by Metrolink to cover their ass so soon after the Chatsworth wreck. An initial report said that only one person was in the cab.
