All that would be necessary is lube oil getting through the intake valves, and since there is a turbocharger on the intake manifold .. .. .. It could also happen with blow by, which seems a lot less likely. Don't know about the wrist pin bearing "coming loose" (wrist pin slap sounds slightly different than rod knock), but that incident must have had a hole in the piston crown somewhere.

Not being that familiar with these sorts of details with Detroits, I've emailed a freind who has been a certified GM mechanic for 50 years. I'll post his answer. BTW, an apostrophe indicates possessive not plural -- "an EMD's injector is a unit injector". Plural: "three EMDs, all alike except for color".
