All I can offer in response is that, yes, if you look through the two books I've had a hand in, there are errors that crept by our (or my) notice, and many- especially in the first book- are due to me only having one shot at the proofs and the publisher quite frankly screwing up my edits and comments to the set of proofs I saw.
Second, I'd tend to separate production issues- such as the pages sticking together- from whatever poor job in either writing and/or editing the book that happened well before it went to print.
Finally, by the time the book is in print, by that time it's really too late for someone who may actually know about what they are talking to become involved in the process...you may have a historical society or members thereof finding errors in a book, and if I was in the publisher's shoes I think I'd be blowing them off as well, as by the time the mistakes have been found they have been committed, and it's really too late to ammend or correct. Your only hope is that the publisher agrees to do an updated edition somewhere down the road, but in today's publishing environment that's a tall order.
