Losing location, distraction, sleep, are all possibilities..... and worse we may never really know, except to rule out mechanical malfunction, which then only leaves human error as the "most" likely cause.
It is increasingly clear that he accelerated very rapidly in that one minute lead up, why, we may never know. But, I am still shocked that 6 years after Chatsworth, there still was no inward facing camera, on these new engines.
I managed several transit technology projects that put up to 8 cameras in on transit buses.... forward facing, sides, inside rear, driver facing, back to front, door facing..... when I was doing transit tracking and system integration projects 10 years ago.
These on-board camera systems and data storage have mainly had the effect of saving transit operators millions of dollars in false accident liability claims, by documenting how and why bus accidents occurred. It has also allowed law enforcement to arrest and convict hundreds of bullies, robbers, mashers, and other criminals on buses.
Now, like all of the RR rules which have been "written in blood", we will finally have inward facing cameras.
