An inward facing camera may well have prevented this accident, if there was inappropriate activity, by the engineer, that contributed to this accident. If not, then no, it would not have prevented the accident. But we'd at least have a record of what he may have done wrong. Since we didn't have it, we will never fully know, what he may have been doing.
And yes, an inward facing camera would almost certainly have helped to prevent what happened at Chatsworth, as Sanchez would have been forced to text in front of a live camera.
Although the grossly incompetent operator under contract to Metrolink and responsible for supervising Sanchez, Veolia, may well have blown off that monitoring requirement, like they did other requirement. Or, like the complaints they received about Sanchez from fellow crewmembers, they could have just continued too ignore his willfully dangerous conduct?
A properly integrated PTC system should do much better in preventing exactly this kind of failure.... But, since we have decided, for now, to leave the "human" in the loop, to operate the train, then monitoring, at least to let us know more about the cause, and how to prevent it in the future, will now be required.
And, I've seen plenty of unsafe bus drivers fired for cause, because they were caught texting while driving on camera.
