The reference is to the collision between an eastbound BNSF intermodal train and a westbound Metrolink train at Atwood (located in Placentia) on April 23, 2002.

The NTSB determined the cause of the collision was the BNSF crew's failure to operate their train in accordance with a APPROACH indication, which they apparently overlooked.
